Hearthstone is expensive to get in to. While at it's core the game is completely free, you are put at a severe disadvantage compared to players who do pay or have been playing for a long time. You're missing out on large parts of the game if you do not own the correct cards. You can obtain all cards without every paying, but that requires you to play the game for a very large amount of time. New expansions release regularly, with this some of your cards might become irrelevant. Either because they are no longer as strong, or because they are straight up taken out of the main game. Cards of some modes are not compatible with cards in other modes, requiring you to collect more and more cards.
Cards are not the only thing you can purchase. There are more cosmetics available. For example different card backs or different hero appearances. Some of these can be earned for free, but often only during a limited amount of times. Others need to be purchased, often not directly but through bundles.
There's also several single-player adventure modes available that require a purchase to enter. Purchasing all of these will cost you over 150 USD.
Some modes require a purchase every time you play, but can be purchased with in-game earned currencies as well.
The game isn't strictly pay-to-win, but if you are a new player and you don't want to spend any money you're going to have a rough time.
